Benefit was strongest in people with limb-onset disease

The benefit was particularly notable among people with limb-onset, or spinal-onset, ALS, whose symptoms typically begin in the limbs. Median survival from symptom onset without invasive ventilation was 44 months with specialized care, compared with 25 months with general neurological care.
The researchers suggested that the survival benefit was unlikely to be explained by any single treatment. Instead, it may reflect the \u201ccumulative effect of coordinated multidisciplinary management,\u201d including regular follow-up, early recognition of complications, and coordinated decisions among healthcare professionals, patients, and caregivers.
The study, \u201cImplementation of specialised care improves survival in patients with amyotrophic lateral sclerosis<\/a>,\u201d was published in Neurological Research and Practice<\/em>.
ALS has a highly variable course. In some people, symptoms<\/a> begin with weakness in the limbs, while in others they first affect speech, swallowing, or breathing.
Specialized multidisciplinary care may also affect outcomes. It can include rehabilitation, respiratory support, and nutritional management and has been associated with better outcomes in ALS. However, real-world evidence comparing general neurological care with structured, specialized care remains limited.
To investigate this, researchers at the Medical University of Vienna in Austria conducted a retrospective study comparing patients treated before and after a specialized ALS outpatient clinic was established in 2018. They examined survival, healthcare use, and treatment patterns to explore which aspects of multidisciplinary care might have contributed to differences between the groups.
The study included 242 people with ALS, of whom 106 received general neurological care and 136 received specialized multidisciplinary care. Their mean age at disease onset was about 61 years.
About two-thirds of participants had limb-onset ALS, meaning their first symptoms affected the arms or legs. About one-third had bulbar or respiratory onset, with symptoms initially affecting speech or swallowing, or breathing.
Disease stage at diagnosis did not differ significantly between the two groups. Use of treatments such as riluzole (brand names include Rilutek<\/a> and\u00a0Tiglutik<\/a>; generic versions are also available), feeding tubes, and invasive and noninvasive breathing support was also similar between the groups.
However, the median time from the first symptoms to ALS diagnosis<\/a> was significantly longer among those receiving specialized care than among participants receiving general care (11 vs. 9 months).
Of 178 people with available data, those receiving specialized care had more hospital stays each year than patients on general care, with a median of 3.4 versus 1.2. However, the groups did not differ significantly in the number of medication groups prescribed each year or in the use of physiotherapy, occupational therapy, or speech therapy.
By the end of the study, 102 people in the general care group and 104 in the specialized care group had died. Median survival from symptom onset without invasive ventilation was significantly longer among those receiving specialized care than among patients receiving general care (40 vs. 27 months).
The difference was particularly pronounced among people with limb-onset ALS, with a median survival of 44 months with specialized care, compared with 25 months with general care. No significant survival difference was seen in the bulbar\/respiratory-onset group.
Additional analyses examined survival from diagnosis rather than symptom onset. Among people with limb-onset ALS, median survival was 29 months with specialized care and 16.5 months with general care.
The survival advantage associated with specialized care was most pronounced during the first two years after diagnosis and was no longer significant thereafter. There was also no significant association between diagnostic delay and survival.
The researchers noted that the retrospective study compared patients treated during different time periods, so changes in ALS care over time or other unmeasured differences between the groups could have influenced the results.
The researchers noted that the findings \u201cmake it unlikely that the observed survival benefit is explained by the longer diagnostic delay\u201d in the specialized care group, suggesting that specialized multidisciplinary care \u201cexerts its greatest survival benefit early after diagnosis.\u201d
